The United faithful went into a frenzy as two extra-time goals delivered the club their first premiership since 1993.
Substitute striker Daniel McKenzie made the most of a Fred Gardner cross to put Leeton United 2-1 up five minutes into the first half of extra-time.
It was then Fred Gardner who delivered the sealer three minutes into the second half as he worked past the keeper and found the back of the net.
It completed a brilliant season from Leeton United, who gradually built momentum as the year went on, culminating in historic finals campaign.
Leeton United coach Frank Alampi struggled to describe what the win meant to the club.
"I don't know what (the feeling) is. I think it's relief and excitement all mixed up together," Alampi said.
"I'm just so happy for the boys, the supporters, ex-presidents, ex-players, sponsors, coaches, not just this year but everyone who has been on the journey to get here. It's not just these boys, it's a whole lot of them to get here, it's been 27 years of hard work from so many different people. It's absolutely effing brilliant. It's just magic."
Earlier, the game went into extra-time after the scores were locked 1-1 at full-time.
Adam Raso gave Leeton United the lead in the 23rd minute when he produced the perfect free kick from just outside the box.
It was almost that exact spot that Michael Musitano scored from his free kick to level the scores in the 63rd minute.
Alampi said it was a deserved win for his team.
"We deserved it. We were the best team all year," he said.
"We didn't play well today, we didn't, but Hanwood caused that. But when we did play well, we played well and really knocked it about."
It was not a grand final for the ages in terms of a spectacle. But the game really came to life late, particularly after Hanwood levelled and then appeared to be finishing the better.
Alampi loved the resilience showed by his men.
"We didn't start well. We didn't start well at all," he said.
"Raso got a goal against the run of play and that settled us down a bit. I think their goal was probably against the run of play as well. Then they got on top of us a little bit but you know what, the boys bounced back."
Leeton goalkeeper and co-captain Jarrod Sillis was named Player of the Match.
Pablo Quarin played the game of his life and the pressure he applied on the opposition was absolutely outstanding for Leeton, while Fred Gardner stepped up in extra-time to help deliver both goals.
